Register with UHS - Dependants of UoS students
Key information when completing the registration form.
Address
Dependants (spouse/partner/child only) of students must live within our practice's inner boundary.
To prevent delays and ensure we have your full address you should select an address from the pre-generated list with the registration form. You see this after you have entered your postcode.
Some accommodation buildings may have multiple postcodes, so make sure you enter the correct one. You can check this:
- On your tenancy agreement
- Via
Your address will not appear in the pre-generated list if you enter the wrong postcode in the registration form.
If you are unable to find your correct address in the pre-generated list, you can enter your address manually. When doing this it is very important to ensure you state your full address, including: room number (if applicable), flat number, block number / letter, building name / number, street name and number etc..

Registration forms submitted between 14th August and 18th September will not be processed until 19th September. This is so we can manage a high volume of registrations ahead of the new academic year. If you have pre-registered and need our services before 19th September you should contact us so we can process your registration sooner.
Forms submitted outside of this period are processed as soon as possible (usually within a week).
